To make pre-planned or immediate arrangements please complete the following 3 forms with all of the pertinent information that is required. The first 2 forms are highlighted in all of the required areas that need to be completed. Form 3 only requires a printed name at the top and a signature and date in the space provided, the funeral home will check the necessary boxes for completion. Once completed and signed please email the forms back to the Funeral Home at info@jbcares.com or fax them to us at (484)893-8790. Be sure to include your name and contact number in the email as well. After all paperwork is reviewed by our office we will reach out to finalize all arrangements as well as arrange for payment.
